What Is O-desmethyltramadol?

O-desmethyltramadol, also known as O-DMST, Desmetramadol, and M1, is an opioid analgesic belonging to a group of compounds known as Cyclohexanol. This class of compounds is characterized by having a cyclohexane bound to a phenol group.

Interest in O-desmethyltramadol increased significantly once it was shown to be an active metabolite of Tramadol, a commonly prescribed pain medication. Tramadol is demethylated by the CYP2D6 liver enzyme much the same way codeine is. Because of this, O-desmethyltramadol is said to cause similar effects to codeine when ingested. It is also worth noting that O-desmethyltramadol is significantly stronger than its parent molecule Tramadol.

Although there has only been limited study on the effects of O-desmethyltramadol in humans, it shares many chemical properties with Tramadol and Codeine, two substances which have been studied to a much greater degree of depth.

Because of this, it can be reasonably assumed that O-desmethyltramadol works as a serotonin and noradrenaline reuptake inhibitor and acts upon the u-opioid receptors. Unlike Tramadol, O-desmethyltramadol has a comparatively high affinity for the k-opioid receptors, which are responsible for feelings of sedation, dissociation, and delirium.

Due to the minimal amount of human study conducted, exact dosage rates are not available. However, anecdotal evidence suggests that a threshold dose is somewhere around 15mg, with 30 - 50 mg being a more common dosage.

As with other opioids, onset usually begins in as little as 30 minutes, with peak effects occurring between 2 and 5 hours after ingestions..

Constipation: The Gastrointestinal (GI) tract contains several neuron clusters that control muscles in the intestines that are responsible for pushing stool towards the rectum. When one ingests opioids, these muscles relax, slow down, and even sometimes stop working altogether. When this occurs, the user is left with a strong sense of constipation.
